GP-led clinical pharmacy

Clinical pharmacy. Managed end to end.

PCS provides the clinical capacity and the operating model behind it: from named pharmacists and supervision to mobilisation, governance and reporting across practices, PCNs and wider NHS programmes.

One managed serviceLocal delivery. Clear accountability.
Flexible by design

Two ways to add clinical capacity.

Use PCS for patient-facing clinical work by telephone or video, day-to-day medicines management, or both. We shape capacity around the work your teams need covered.

01 / CLINICAL PHARMACY

Remote patient-facing clinical care

Structured pharmacist-led consultations delivered by telephone or video, adding clinical capacity without losing clear supervision or escalation.

  • Structured medication reviews
  • Medication review consultations
  • Chronic disease reviews
  • Extended access and out-of-hours consultations
02 / MEDICINES MANAGEMENT

Medicines management

Day-to-day prescribing and medication workload handled to agreed protocols, freeing practice clinicians for work that needs them.

  • Prescription and script support
  • Medication review from clinical notes
  • Practice-specific medicines workflows
  • Clinical queries managed within agreed scope
PCS PCN operating model

One network. One accountable pharmacy service.

PCS manages the service at network level while named pharmacists work within each practice’s systems, pathways and priorities: one operating model with local continuity.

01

Set priorities

Agree priorities, capacity, practice allocations and success measures.

→
02

Embed locally

Named pharmacists work within each practice’s systems, formulary, protocols and escalation routes.

→
03

Flex capacity

Shared capacity can move to peaks, targeted cohorts and network-wide clinics.

→
04

Review & rebalance

Review utilisation and delivery, then rebalance through the agreed PCN governance route.

Three commissioning models

Choose how capacity is deployed.

MODEL A

Practice pods

Named pharmacist capacity is assigned to practices for consistent weekly delivery, while PCS manages supervision and performance centrally.

  • Predictable weekly capacity
  • Named practice relationships
  • Local workflows retained
MODEL B

PCN hub

A pooled pharmacy team works across the network against agreed cohorts, clinics and medicines workload, directing capacity where needed.

  • Network-wide programmes
  • Central remote-clinic capacity
  • Capacity follows demand
2026/27 ARRS service arrangements

Where ARRS reimbursement is intended, PCS can structure the service around current Network Contract DES requirements, including role scope, qualifications, MDT integration, continuity and supervision. Alternative-provider arrangements should be intended to run for at least six months. Eligibility and reimbursement remain for the PCN and commissioner to confirm.

ICB & place-based programmes

Commission once. Deliver across the system.

PCS can deliver one pharmacy specification across multiple PCNs and practices, with consistent governance and reporting while preserving local formularies, systems and escalation routes.

01 / DEFINE

Set the system brief

Agree the population, clinical scope, priorities, footprint and measures once.

02 / DEPLOY

Deliver locally

Deploy through participating PCNs and practices, configured to local workflows.

03 / SEE

Keep one line of sight

Review at system level with drill-down to PCN and practice activity, quality and utilisation.

One programme. Local delivery. Comparable data.Useful for targeted medicines optimisation, variation reduction, access programmes or time-limited capacity across a wider footprint.

Value & productivity

Transparent capacity, utilisation and agreed measures.

03

Integration & sustainability

05

CQC medicines readiness

Support practice preparation around medicines reviews, monitoring, safety alerts, prescribing audits, Controlled Drugs, SOPs and action tracking.

06

Social value

Procurement-specific social-value commitments can be agreed and reported.

Commissioner assurance pack

For procurement or mobilisation, PCS can provide an evidence pack proportionate to the commissioned model.

Workforce registration, competency, prescribing status where relevant, checks and indemnity evidence.
Clinical governance, supervision, escalation, incident, complaints and audit arrangements.
Information-governance evidence, data-processing arrangements and access model.
Business continuity, named cover and capacity-contingency arrangements.
Service specification, KPI schedule, mobilisation plan and reporting definitions.
Documented governance scope, responsibilities and escalation arrangements for the specific delivery model.
Evidence supplied reflects the commissioned model. CQC-readiness support relates to medicines governance, records, audits and processes; physical site checks remain the practice’s responsibility.
How we start

From plan to live service.

We agree the commissioning model first, then configure local delivery, governance, access and reporting before clinical activity begins.

01

Shape the model

Agree footprint, capacity, clinical scope, expected activity, measures and intended funding route.

02

Map each practice

Confirm systems, formularies, protocols, prescribing boundaries and escalation routes.

03

Build the team

Set named allocation, shared capacity, consultation structure, professional supervision and GP oversight where required.

04

Launch & optimise

Go live, review delivery and utilisation, then adjust capacity through the agreed governance route.

Bring us the brief. We’ll build the service.

Practice, PCN or system-wide programme: share the footprint, priorities and capacity challenge. We’ll map the delivery model, governance and resource needed.
